We offer three choices for breast mound reconstruction in Atlanta. Any of these procedures can be performed at the time of your mastectomy.
- The first choice is a two-stage procedure involving Tissue Expander and Breast Implants. In the first stage, we place the Tissue Expander at the time of mastectomy. Saline fluid is added weekly to the Expander until the desired size is reached. At the second stage, we subsequently replace the Tissue Expander with a Breast Implant.
- The second choice is a Latissimus Dorsi Myocutaneous Flap, with or without an implant. This technique harvests muscle and skin from your back and transfers it to the mastectomy site to replace the missing skin. A breast implant is used to replace the missing volume. The breast mound is completed during this stage, commonly performed at the time of the mastectomy.
- The third choice is a TRAM: a Transverse Rectus Abdominis Myocutaneous Flap. A TRAM is a flap of skin, fat, and muscle harvested from your abdomen (belly). This technique does not require a breast implant. The donor site tightens, reducing the size of your abdomen. The breast mound is reconstructed in one stage commonly performed at the time of the mastectomy. This is a bigger surgery than the other two choices. It requires a 3–5 day stay in the hospital and a donation of at least two units of your own blood to be held in reserve for a transfusion after the surgery.

Regardless of the breast mound technique, we will reconstruct your nipple in a separate procedure. A local skin flap is created on the breast mound to fashion a nipple. An Alloderm graft (an acellular dermal matrix) is used to give volume to the reconstructed nipple. The areola is reconstructed with intra-dermal tattooing of the pigment after the nipple reconstruction has healed.
