How to Apply for CareCredit

You can apply for CareCredit in one of three ways. Use the method that is easiest for you.

Standard Application

1. Ask the office staff for an application.
2. Complete the application form.
3. Return it to the office staff for processing.
4. When approved, contact the office to schedule your care.

Online Application

1. Go to www.carecredit.com.
2. Fill out the patient application.
3. Receive a quick credit decision.
4. Contact the office to schedule your care.

Automated Phone Application

2. Follow the prompts.
